We are a California Department of Health Care Services PATH Technical Assistance Provider
Providing Access and Transforming Health (PATH) supports California community-based organizations (CBOs), hospitals, county agencies, Medi-Cal Tribal entities, and Indian Health Program designees, among others, in actively engaging with the Medi-Cal delivery system. This support is essential to expanding Enhanced Care Management (ECM), Community Supports, and Justice Involved services under the California Advancing and Improving Medi-Cal (CalAIM) initiative.
The goal of PATH is to advance health equity by addressing social determinants of health, creating a more coordinated, equitable, and accessible Medi-Cal system that serves the diverse needs of Californians.
technical assistance Marketplace
The Technical Assistance (TA) Marketplace initiative offers funding to providers, community-based organizations, counties, and others to access the technical assistance resources necessary for implementing Enhanced Care Management (ECM) and Community Supports.

1. Eligibility
Confirm that your organization qualifies as an eligible entity type (e.g., CBO, local government agency, FQHC, Medi-Cal Tribal entity, Indian Health Program designee, or approved provider).
Determine the specific eligibility category that applies to your organization.

2. Application
Collect all necessary supporting documents. Access the application form and provide the required information along with supporting documents to confirm your organization's eligibility.
Submit the completed TA Recipient Eligibility Application. If you don’t have an account on the platform, you will be prompted to create one during the process.

3. Project Approval
After you become an approved TA recipient, you will be required to submit a technical assistance Project Eligibility Application (PEA).
We will work together to develop a strong PEA that reflects your immediate and near term goals, while ensuring the application content and structure enhances a timely approval by PATH TA Marketplace administrators.

4. Get Started
With the approval of your Project Eligibility Application, we will handle the preparation and submission of the project Scope of Work and Budget Proposal on your behalf. We look forward to collaborating with you!

Eligible entities encompass Community-Based Organizations, Government Agencies, Federally Qualified Health Centers, Medi-Cal Tribal programs, and others approved by DHCS. To qualify, entities must meet criteria outlined in one of three options. 1) be contracted with a managed care plan 2) be planning to contract with a managed care plan, or 3) be approved by DHCS.

To access TA resources through the TA Marketplace, organizations need to submit a TA Recipient Eligibility Application, which DHCS will review to confirm eligibility.
