PATH Technical Assistance Provider

Providing Access and Transforming Health (PATH) supports California community-based organizations (CBOs), hospitals, county agencies, Medi-Cal Tribal entities, and Indian Health Program designees, among others, in actively engaging with the Medi-Cal delivery system. This support is essential to expanding Enhanced Care Management (ECM), Community Supports, and Justice Involved services under the California Advancing and Improving Medi-Cal (CalAIM) initiative.

The goal of PATH is to advance health equity by addressing social determinants of health, creating a more coordinated, equitable, and accessible Medi-Cal system that serves the diverse needs of Californians.

  • Domain: Promoting Health Equity

    We design bespoke health equity programs by developing targeted population health strategies. We encourage organizations to lean into their values while leveraging available data to define their own health equity goals. We then provide a roadmap and training necessary to foster change management that leads to measurable progress in health equity and long-term improvements in population health.

    We counsel organizations to overcome these challenges by providing recruitment, retention, and growth plans for frontline, clinical, and/or “lived experience” workforces. We guide leaders through the process of creating clear roles, improving management practices, and advancing the skills of the community health workforce, ensuring that your team is not only sustainable but also well-equipped to drive positive health outcomes in the communities you serve.

    We enable organizations to harness their data effectively. From comprehensive documentation of social needs screening to spatial analysis using Geographic Health Information Systems (GHIS), we help you identify gaps, plan interventions, and measure your progress in reducing disparities and meeting equity goals. We believe in data democratization, providing the organizational insight necessary to drive informed, inclusive decision-making.

The Technical Assistance (TA) Marketplace initiative offers funding to providers, community-based organizations, counties, and others to access the technical assistance resources necessary for implementing Enhanced Care Management (ECM) and Community Supports.

How The Technical Assistance Marketplace Works

  • 1. Eligibility

    Confirm that your organization qualifies as an eligible entity type (e.g., CBO, local government agency, FQHC, Medi-Cal Tribal entity, Indian Health Program designee, or approved provider).

    Determine the specific eligibility category that applies to your organization.

  • 2. Application

    Collect all necessary supporting documents. Access the application form and provide the required information along with supporting documents to confirm your organization's eligibility.

    Submit the completed TA Recipient Eligibility Application. If you don’t have an account on the platform, you will be prompted to create one during the process.

  • 3. Services

    After your eligibility is confirmed, log in to the platform and explore Pivot First Consulting and Management’s available Technical Assistance (TA) services.

    Select the TA service(s) that best matches your organization’s needs. We provide tailored, hands-on technical support. You may also be required to submit a Technical Assistance project eligibility application to DHCS.

  • 4. Get Started

    Pivot First Consulting and Management will handle the preparation and submission of the Scope of Work and Budget proposals on your behalf, ensuring everything is submitted for approval.

    Once the project is approved, you’ll have access to the contracted services, which may include training, consulting, or the implementation of specific program development and strategic initiatives. We look forward to collaborating with you!

As a former owner and operator of a home health agency serving low-income clients with complex needs, Kussy has firsthand experience as a community-based provider, giving her an intimate understanding of the financial, operational, and strategic challenges faced by Medi-Cal providers. She has a proven track record in navigating the intricacies of Medi-Cal expansion for large safety-net hospital systems by creating roadmaps to adopt processes to align care delivery with state and federal policy changes. She believes that Medicaid transformation should translate to enhanced access to culturally responsive and necessary care for everyone.  Kussy is dedicated to empowering clients through data-driven strategies, ensuring they thrive in the evolving Medi-Cal landscape.

  • Eligible entities encompass Community-Based Organizations, Government Agencies, Federally Qualified Health Centers, Medi-Cal Tribal programs, and others approved by DHCS. To qualify, entities must meet criteria outlined in one of three options. 1) be contracted with a managed care plan 2) be planning to contract with a managed care plan, or 3) be approved by DHCS.
